This distinction sounds academic and has real consequences. Is Your Size Currently Stable?
If you are pregnant, lactating, going through a hormonal life stage, changing weight, or in any period of fluctuation, wire-free is the more practical default. Not because a wire is harmful, but because a wire requires accurate fit and accurate fit is difficult to maintain against a moving target. Flexible wire is a reasonable middle option.
What Are You Wearing Over It?
Under thin fitted knits, a moulded wired cup gives the smoothest line, because it holds one fixed shape and has no seams to print. Under woven fabrics, looser clothing, or anything with texture, the difference is negligible and other considerations should decide.
If Wire-Free, What Is the Construction?
This is where people go wrong. Check for seamed rather than single-layer cups, side support panels, a band of two inches or more with a tall wing, and straps an inch or wider. A wire-free bra with those features supports well into the G and H range. One without them is a comfort garment, which is a fine thing to be and not a substitute.
Fit the Band First
The band should sit level, firm, and fasten on the loosest hook when new. If it rides up or feels loose, nothing above it will work properly, and this is true of both constructions equally. The Underwire and Cancer Claim

The claim that underwire bras cause breast cancer originated in a 1995 book by a husband-and-wife team, which proposed that bras restrict lymphatic drainage. It was not a controlled study, and its methodology has been widely criticised for not accounting for known risk factors.
Research examining bra wearing and breast cancer risk has not found a link. Major cancer organisations do not list bra wearing as a risk factor. The claim has nonetheless proved extremely durable online, which is a reasonable illustration of how a confident book can outlast the evidence against it.
A poorly fitted wire pressing into breast tissue is uncomfortable and, for anyone lactating, sustained pressure on the breast can contribute to blocked ducts. That is a genuine reason to insist on correct fit and to prefer wire-free during breastfeeding. It is a fit issue, not a disease issue. Any lump, persistent pain, skin change or nipple discharge should be assessed by a doctor regardless of what you wear.
People arrive convinced they have to choose between support and comfort. They do not. The band supports and the wire shapes, and once that lands the decision gets a lot easier.
