What "40D" Actually Means
A bra size is two measurements read together. The band — the 40 — is the circumference of your ribcage directly under the bust, rounded to the nearest even inch. The cup — the D — is not a volume in its own right; it is the difference between your bust at its fullest and that band. Four inches of difference is a D cup. So a 40D describes a bust roughly four inches fuller than a 40-inch ribcage.
Because the cup letter is relative to the band, the same letter holds more volume on a wider frame. A 40D holds noticeably more than a 34D — around 810 mL per breast against 600 — even though both are "D" cups. The letter has not changed; the frame it sits on has.

Sister Sizing Is More Useful Here Than Almost Anywhere
When a style you want stops at 38, 38DD is your sister size — the identical cup volume on a snugger band. It is not a compromise; it is the same cup. Whether it works depends only on whether a 38 band fits your ribcage, which for many 40D wearers it does, since bands stretch and most people buy them too loose to begin with. Try the 38DD before assuming the style is unavailable.
Band Construction Matters More Than Cup Shape
A wider band has more length to stretch, more surface area to roll, and more weight travelling along it. At 40D the difference between a two-hook band and a four-hook band, or a three-inch wing and a five-inch wing, is far more noticeable than the difference between a moulded and a seamed cup. Prioritise band engineering when choosing between two otherwise similar bras.
This matters more at a 40 band than at any smaller size. A longer band has more elastic in it, so it loses tension faster. Buy so the bra fits correctly on the loosest hook and tighten inward as it wears in. A 40D bought on the tightest hook has nowhere to go and will feel loose within weeks.

A cup letter describes a ratio between bust and band rather than an absolute volume, so a D on a 30 band and a D on a 44 band are proportionally comparable but hold quite different amounts of tissue.
